Three simple steps to make the most of your first visit to Sweetwater Pain & Spine

Following the steps below ensures we can make the most of your appointment with us.

Complete Your Paperwork

Arrive 30 min early

If you have not completed the paperwork in advance, please plan to arrive 30 minutes before your scheduled appointment time to fill it out at the front desk.

Arrive 15 min early

If your paperwork is already completed and printed, arriving 15 minutes early is sufficient to check in and get settled before your appointment begins.

Bring These Documents

Having the right documents at your first visit allows your physician to begin a thorough evaluation without delay. Please gather the following before your appointment:

Driver’s license, passport, or other government-issued photo identification

💳 Insurance Card

All active insurance cards, including primary and secondary coverage

📁 Medical Records

Prior imaging, surgical notes, specialist letters, and test results relevant to your pain

Current Medication List

Bring a complete list of all medications you take, including dosages — prescription, over-the-counter, vitamins, and supplements. Alternatively, bring the bottles themselves.

Prior Imaging (MRI, X-Ray, CT)

If you have prior imaging studies, bring the discs or printed reports if possible. Your physician will review these during your visit. If you cannot obtain them in advance, let our office know and we can assist with records requests.

Prior Treatment Records

Any notes or summaries from previous pain specialists, surgeons, neurologists, or physical therapists are extremely helpful — even if treatments were not successful. Knowing what has already been tried guides your new evaluation.

Be prepared to share any known drug, latex, or contrast dye allergies, including the nature of the reaction. This information is critical before any procedures are performed.

What to Expect at Your Consultation

Your initial consultation typically lasts approximately 30 minutes and is conducted by one of our fellowship-trained, board-certified pain medicine physicians. Here is how your visit will unfold:

Check-In and Records Review

Our front desk team will verify your insurance, collect your documents, and ensure your paperwork is complete. Your physician will review your medical history, prior records, and imaging before entering the room.

Detailed Pain History

Your physician will ask detailed questions about your pain — where it is, when it started, what makes it better or worse, how it affects your daily life, sleep, and function, and what treatments you have already tried. There are no wrong answers; the more detail you can provide, the better.

Focused Physical Examination

A targeted physical and neurological exam will be performed, evaluating range of motion, strength, sensation, reflexes, and specific pain provocation maneuvers relevant to your condition.

Imaging and Records Review

Your physician will review any MRI, X-ray, or CT images you bring and explain what the findings mean in the context of your symptoms.

Diagnosis and Treatment Plan

At the conclusion of your visit, your physician will share their assessment, discuss the likely cause of your pain, and outline a recommended treatment plan. This may include additional diagnostic testing, medications, physical therapy, interventional procedures, or a combination of approaches. You will have time to ask questions before you leave.

A note about medical scribes: Our clinic utilizes trained medical scribes during most visits. A scribe assists the physician by handling electronic medical record documentation in real time — allowing your doctor to remain fully focused on you during the entire appointment rather than on a computer screen. The scribe is bound by the same confidentiality standards as all clinical staff.

Frequently Asked Questions

In most cases, no. Your first appointment is a consultation — a thorough evaluation to understand the nature and source of your pain. Procedures and injections are scheduled at a separate appointment after your physician has completed a proper assessment and determined the most appropriate treatment plan for your condition.
Plan for approximately 45–60 minutes total for your first visit, including check-in, paperwork (if not completed in advance), the consultation itself (typically about 30 minutes), and any time needed to schedule follow-up appointments or tests before you leave.
It depends on your insurance plan. Some plans require a referral from your primary care physician before seeing a specialist; others do not. We recommend checking with your insurance carrier before your appointment. Our office can also help you verify your benefits when you call to schedule.
Please still come to your appointment. While prior records and imaging are helpful, they are not always required for your initial evaluation. Let our team know when you arrive and we can assist with records requests on your behalf. Imaging can often be ordered at your visit if needed.
Yes, and we encourage it. Having a trusted person with you can help you remember details during the history-taking, take notes when treatment options are discussed, and provide emotional support. Please notify our front desk when you arrive if someone will be joining you in the exam room.
Before you leave, your physician will outline the next steps — which may include ordering additional imaging or diagnostic tests, starting a medication, arranging a referral for physical therapy, or scheduling a procedure. Our scheduling team will help coordinate any follow-up appointments before you leave the office. If additional testing is ordered, we will reach out once results are available to discuss next steps.
